DM Care empowers diabetes patients
In order to allow better disease management, a new mobile application DM Care will be available for diabetes patients. With the app, patients can record and monitor blood glucose level through charting, and in turn control the disease and maintain a healthy diet. In follow-up consultation, doctor, nurse and dietitian will review together with the patient the blood glucose and diet record, thus providing more appropriate medical advice. Patients will also receive reminders from the app when it is time to measure blood glucose level, take medicines and visit the doctor.
In addition, the app provides useful information and videos on diabetes mellitus, including prevention methods, oral care and foot care, helping carers learn skills to take care of patients. A pilot scheme is now rolled out in New Territories East Cluster’s hospitals including Prince of Wales Hospital, Alice Ho Miu Ling Nethersole Hospital, North District Hospital and general out-patient clinics. The app will be available for download on app stores in mid-October.
