Introduction

The aims of Patient Resources Centre of Tin Shui Wai Hospital are to promote patient health empowerment service and encourage community engagement in patient rehabilitation process. The services include primary health education, disease prevention and patient support services. As a bridge between the community and the patient, the Patient Resouces Centre creates a platform for information sharing. It provides community resources to patients and carergivers to enhance disease self-management, health information to community to support a healthy community living.

Mission

The Patient Resources Centre is committed in promoting holistic health care to patients and their caregivers in physical, mental and social well-being. Through patient empowerment, patients’ knowledge of disease management,self-care capacity, competency in rehabilitation is strengthened. The improvement of quality of life is enhanced as the result.

Besides, the Patient Resources Centre collaborates with community organizations in promoting community support to patients and to build a caring community.

Service Objective

  1. To provide health education and psychosocial support services to patients and caregivers;
  2. To encourage community resources sharing, to advocate healthy community concepts and to achieve the goal of community care.
  3. To build a mutual support platform and to provide empowerment activities for patients and caregivers so to enhance their disease self-management and confidence.
  4. To facilitate the participation of patients, caregivers, volunteers and community groups,to promote community health and enhance self-management of chronic diseases.

Service scope

  1. Empowerment and Support to Patients and Caregivers: To provide patient empowerment and patient health education activities, psychosocial support and related support activities.
  2. Support to Patient Groups : To support patient groups and to provide peer support for people facing similar diseases.
  3. Volunteer Service and Development: To coordinate,develop and manage hospital volunteer services, so to provide support to patient and hospital services.
  4. Community Engagement and Partership : To collaborate with community partners, such as NGOs, schools,etc, to promote healthy lifestyles and patient support services.

Target

Inpatients in Tin Shui Wai Hospital, outpatients of NTWC hospitals, hospital staff, individual communtiy members and the social service organizations in Tin Shui Wai, Yuen Long and Tuen Mun district.

Service Hours

Monday to Friday 9:00 am to 1:00 pm
2:00 pm to 5:00 pm
Saturday, Sunday and Public Holiday Closed
