Booking Arrangement for New Case
The required documents for new case booking are as follows:
- Valid referral letter (except EASY Service),
- Valid identity documents and
- Proof of residential address e.g. electricity, gas or water bill
Notice:
- Referral for new case booking is valid for 3 months from the date of issue.
- For the booking arrangement of EASY new case, please contact 2928 3283.
Notice to Patient upon Consultation (New case)
- Patients should attend the centre according to the scheduled date and time, with their appointment slip.
- Please present the following documents upon registration:
- Valid identity documents
- Civil servants, retired civil servants and their eligible family members; HA staff, retired HA staff and their eligible family members must inform counter staff of their eligibility and provide valid proof of their eligibility for medical benefits.
- Related medical references (X-ray Film or / and Lab. Result) if any
- For rescheduling or defaulting cases, patients may call the relevant specialty or visit respective clinic in person for rearrangement.
In Case of change of Mental Condition Before the First Appointment
Should an urgent condition arise before the appointment date and time, patients may take the following actions:
- Seek medical advice from registered general practitioner / General Out-patient clinic
- Consult the Hospital Authority Mental Health Direct at 2466 7350
- Contact our staff in person or by phone during office hours of the relevant specialty
- Attend private psychiatric consultation.
- Attend the Accident & Emergency Department or call police for help in case of emergency
Change of appointment or Enquiry for Old Case
For old case, follow-up consultation is by appointment only. Rescheduling of appointment may be made in person or by phone with the relevant specialty. Please note that patients’ personal data may be required for all appointment booking.
Dispensing of Medication
Please collect medications within 4 working days; otherwise the prescription sheet would be voided.
