Please return the completed Donation Form and donations (cheque or original bank-in slip) to Prince of Wales Hospital, 30-32 Ngan Shing Street, Shatin, N.T., H.K. Please mark “Prince of Wales Hospital Charitable Foundation” on the envelope. Donation methods include:

1. By Cheque
   Donation by crossed cheque should be made payable to “Prince of Wales Hospital Charitable Foundation”.
2. Deposit donation to Prince of Wales Hospital Charitable Foundation Bank of China bank account. Account No: 012-875-11380805 (For overseas remittance, please refer to the Quick Reference Guide)
