Procedures of end-of-life services
2. Formulate ACP according to the patient and family’s preference. The choices of medical treatment include,
• Cardiopulmonary resuscitation
• Nasogastric tube feeding
• Intravenous or subcutaneous fluid
• Intravenous and / or oral antibiotic
• Non-invasive positive pressure ventilation
3a. RCHE staff will liaise with PWH CGAT for direct admission to SH through Non-Emergency Ambulance Transport Service when EOL patients need hospitalisation. This helps bypass PWH AED and minimise sufferings associated with medical treatment in acute hospital.
3b. Sometimes, immediate medical attention at PWH AED is necessary when EOL patients are critically ill. The attending doctors will deliver treatment in compliance with ACP. The patients are then transferred to SH as early as possible.
4. EOL patients continue to be cared in designated wards at SH.
