1. Patient and Carer Empowerment and Support Service To provide patient empowerment and education programmes, psychosocial and other supports for patient and carers
- Co-organize sharing sessions with patient groups
- Provide education (e.g. health talk)/treatment (e.g. orientation programmes for newlydiagnosed patients)/psychosocial support (e.g. stress management) for patients toenhance their abilities in self-care and disease management
- Establish patient support and resources kiosks
- Concern visits in wards and out- patient clinics
- Peer sharing and support
- Produce publications to enhance patients’ knowledge on disease information and resources

2. Support to Patient Groups To help develop and consolidate patient self- help groups and mutual support network to support patients and carers
- Provide consultation in the establishment of the patient groups and to help in their consolidation process
- Co-organize patient sharing and other activities with clinical team to enhance patients’ confidence and abilities in self-care and disease management
- Provide continuous programmes and support networks to enhance the community support towards discharged patients
3. Volunteer Services and Development Develop, organize and supervise volunteer services to enhance patient care
- Organize and manage volunteer services including recruitment, training and appreciation
- According to the needs of departments, support and supervise both individual and organization volunteers to provide direct and indirect patient support
4. Community Engagement and Partnership To provide platforms to facilitate the participation of community partners and service users to enhance medical social collaboration

5. Health Corner HRC provides a self- help service for measurement of blood pressure and body mass index (BMI). Patients can better assess and monitor their health condition with these facilities. Moreover, patients can also use the computer in HRC to access to the Smart Patient website for disease information and community resources.
