Clinical Information (For Continuation of Care)

• For requests made by non-HA medical practitioner for the purpose of continuation of care, request should be submitted to our hospital in writing, stating the reasons of request and the information required.


• The original / certified true copy of the written authorisation duly signed by the patient, which should include the patient’s name, HKID No. and date of signature, must be submitted with the request.


• According to the Hospital Authority’s policy, no fee will be charged if the information under request is for the purpose of continuation of care.
