Siu Lam Hospital Outreach Service
Introduction
The target service users of Siu Lam Hospital Outreach Service are those registered on the Central Waiting List and outreach list as well as their main carers.
Our missions are:
- To explore available community resources for continuous care of service users in the community.
- To provide appropriate community support to service users for maintenance of their function while waiting for long-term infirmary service in Siu Lam Hospital.
- To arrange respite and infirmary care when needed.
Scope of Services
- To provide community support including on-site assessment, care plan formulation, intervention and progress monitoring for the service users who have been registered on the Central Waiting List of Siu Lam Hospital
- To arrange respite care and infirmary admission for service users registered on the Central Waiting List of Siu Lam Hospital
- To provide pre-admission services to service users registered on the Central Waiting List of Siu Lam Hospital
