Application for Patient Data

You and your authorised person can apply for your Clinical Records, which include:

• Duplicate Medical Records
• Medical Report
• Medical Certificate, Hospitalisation / Attendance Records

You can authorise non-HA medical practitioner to request for your clinical records for the purpose of continuation of care.
