Medical Report

• Medical report is a summary of your health condition and treatment given by our doctors.
• Medical report is written by doctor based on review of patient’s medical records.
• All medical reports are written in English.

Insurance Claim Form

• Requests for completing Insurance Claim Form are processed as Medical Report Request.
• Each Insurance Claim Form will be charged as one Medical Report.
• Please submit the Insurance Claim Form with the Application Form.
• Doctor reserves the right to determine whether to complete the Claim Form or issue a separate report in essay format.

Other Forms

• Requests for completing Form are processed as Medical Report Request.
• Unless otherwise specified in Hospital Authority’s Policy, each form will be charged as one Medical Report.
• Please submit the Form with the Application Form.

Amendment of Medical Report

• Please submit the original copy of medical report with the Application Form for Amendment.
• Amendment is subject to our doctors / hospital’s management final decision.
