Medical Certificate
- Commonly known as “Sick Leave Certificate” or “Attendance Certificate”.
- Medical Certificate is usually issued by the attending doctor during consultation free of charge. The certificate is to certify the date of attendance, hospitalisation period, sick leave granted, follow-up date and period of avoiding heavy physical duty of the patient.
Medical Certificate (Electronic Copy)
- With effect from 6 March 2023, all medical certificates issued by the Hospital Authority fully adopted digital signature with electronic version, to replace traditional manually signed paper certificates.
- The electronic certificate will be automatically stored in “My Record” under the HA mobile application HA Go feature “MyHealth”. Patient can use it for sick leave application or proof of consultation. Patient can print out the electronic certificate if required.

Hospitalisation / Attendance Records
- Patient and his/her authorised representative could apply for hospitalisation / attendance records of the patients.
