Who could apply for deceased patient’s medical report / medical records?

  • Personal representative of the deceased, which includes:
    - Executor with grant of probate
    - Executor appointed by the deceased’s last valid will but without grant of probate
    - Administrator appointed by the letter of administration
  • If deceased patient has no personal representative:
    - Direct relative of the Deceased who has a beneficial interest in the estate of the Deceased, and have applied or intend to apply to the court to be appointed as administrator of the Deceased’s estate.

Special Points to Note:

  • The hospital may ask for additional supporting documents depending on the nature of each application. For details, please refer to the “Deceased Patient’s Medical Reports / Medical Records Copies Application Form”.
  • The hospital reserves the right to decline the application and unless the applicant obtains a court order requiring the disclosure of the deceased’s medical records or medical reports.
