Duplicate Medical Records
Medical Records includes:
- Clinical notes (e.g. Discharge Summary, Hospitalisation Records, A&E Records, Consultation Notes, etc.)
- Laboratory reports (e.g. Blood test, Urine test and other Clinical pathology tests, etc.)
- Diagnostic images & reports (e.g. X-Ray, CT Scan, MRI Scan and Ultrasound Scan, etc.)
- Other Investigation Results (e.g. EEG, ECG, etc.)
Duplication of Medical Records (via Data Access Request (DAR) under Personal Data (Privacy) Ordinance)
- Under the Personal Data (Privacy) Ordinance, a patient or his/her authorised representative could make a DAR to the hospital for a duplicate of his/her medical records.
- The patient, in relation to medical records, must be a living individual.
- Our hospital will reply the requestor in writing within 40 days after receiving the DAR.
Correction of Medical Records (via Data Correction Request (DCR) under Personal Data (Privacy) Ordinance)
- By a Data Correction Request, a patient or his/her authorised representative can ask for correction of the Personal Data which the hospital has supplied in pursuant to a DAR or by other legal means (e.g. HA Go). The Requestor should provide evidence to support the inaccuracies claimed.
- The patient, in relation to medical records, must be a living individual.
- Our hospital will reply the requestor in writing within 40 days after receiving the DCR.
