In Patient Information

Admission Procedures

Patients should register at Hospital's Admission Office. The following documents are required during registration:

1. Identification Documents

Patient Status Identification
Documents
Holder of H.K.I.D. Card ● H.K.I.D. Card / Passport
Child (under 11 years of age) ● H.K. Birth Certificate
● H.K.I.D. Card from father / mother
Spouse of a HKID Card Holder ● Marriage Certificate and
● H.K.I.D. Card from Spouse
● Valid travelling document (if necessary)
Child (under 11 years of age) of a H.K.I.D. Card Holder ● Birth Certificate with father / mother's name and
● H.K.I.D. Card from father or mother
● Valid travelling document (if necessary)
H. A. Staff (serving / retired) or his dependent ● H.K.I.D. Card
Civil Servant / Pensioner or his dependent ● T.R.Y. 447/ Pensioner Card

Public Assistance Receiver ● P.A. Certificate

2. Admission Slip (for scheduled admission)

Hospitalisation

  • Patients should not bring large amount of cash, jewellery etc. during hospitalisation.
  • To protect patient's safety, patients are strongly advised not to leave the ward without notifying the attending nurse or ward staff or the hospital will assume no responsibility for any consequences.
  • Should patient's next-of-kin wish to know the latest medical condition of the patient, a meeting with the doctor can be arranged through the attending nurse.
  • The operation of some medical instruments and equipment may be affected by signals emitted from mobile phones. Please turn off the mobile phones inside wards and designated areas.
  • Smoking is strictly prohibited in the hospital compound.
  • To avoid causing disturbance to others, no photography is allowed inside the wards.
  • Offering advantage to hospital staff is strictly forbidden. Please report to Hospital Chief Executive, General Manager (Administration Services) or ICAC should there be any staff asking for advantage.
  • Hospitals may have different visiting hours. Please check with hospital staff for details. Children under 12 are not allowed to go inside the ward.

Discharge

Transfer procedures

Hospital Authority provides services through hospital clusters. Patients will be transferred from general hospitals to convalescent hospitals for continuous treatment in accordance with their medical condition.

Discharge procedures

Please contact Hospital ward steward or nurse for discharge procedures.

Patients having financial difficulty may contact ward staff or medical social worker to apply for waiver of hospital fees.

Your valuable suggestion or feedback regarding the services provided by the Hospital can help us to improve our quality. Please give your suggestion or opinion to Patient Relations Officer, Ward Manager, Hospital Chief Executive etc.
