For requests made by medical practitioner for the sole purpose of continued care by that medical practitioner, please send the application in writing stating the reason of request and type of information required. Please also enclose the original / certify true copy of patient’s authorization letter with the patient’s name, HKID card number and date included. Upon receiving the request, we will release the information to the requested medical practitioner.

According to the Hospital Authority policy, no fee will be charged if the information released to the medical practitioner is in the form of a medical report.
