New Territories East Cluster - Summer Volunteer Programme 2011
Target: Students who reach the age of 15 and are F.4 or above
Hospital Served: Prince of Wales Hospital, Shatin, Bradbury Hospice, Shatin Cheshire Home
Scope of Services:
1. Provide clerical supportive care services in wards/ clinics, e.g. escort and guidance services.
2. Provide clerical support to departments, e.g. production of health education materials, conducting survey and data input.
3.Home visit, assist in organizing patient programme.
Note: Participants will be randomly arranged to serve in one or two of the above hospitals.  Scope of services may  be different among hospitals, depending on the needs and features of respective hospital.  The services may not include or limit to all of the above.
Service Period: 18/7/2011 to 12/8/2011, Monday to Friday, 9am to 5pm (EXCEPT Public Holidays)

  2. Completed application form with 2 recent photos should be submitted by mail or in person to :-
      Health Resource Centre, 2/F, Day Treatment Block, Prince of Wales Hospital, 30-32 Ngan Shing Street, Shatin.
      If submit in person, please note office hours as below:
      8:45am to 5:30pm, Monday to Friday (Closed during 1pm to 2pm lunch break and on Sat, Sun, public holidays)
Note: (i) Application will not be processed if the form is incomplete or without photos.
          (ii) The organizer reserves all rights for final decision of acceptance of applications, service arrangements, programme details and attendance requirements, etc.
Quota: 100 (Ballot will be drawn if no. of applications exceed the quota.)
Date & Time of Ballot: 15/6/2011 (Wednesday), 3pm in Health Resource Centre, 2/F, Day Treatment Block, Prince of Wales Hospital
                                        (Successful applicants will be informed individually by email on 20/6/2011)
