Insight on Diabetes Foot Management in UK: A Sharing Session on Overseas Corporate Scholarship Programme

Diabetes mellitus currently affects over 400 million people in the world. In Hong Kong, it is estimated that about 10% of the
population are diabetic. Diabetes related complications such as foot ulceration can occur when glycaemic control of diabetic
patient is suboptimal. Previous studies have also shown that the five year mortality rate in diabetic patients with new onset of
foot ulceration is nearly 50%, and up to 74% in patients who previously suffered from a lower limb amputation (Robbins et al,
2008).

In 2015, an Overseas Corporate Scholarship Training Programme on diabetic foot management for podiatrists was held in the
United Kingdom. This programme consisted of a five-day specialist course for podiatrists followed by a clinical attachment in
Leicester. Well-established clinical pathway and closely coordinated multidisciplinary relationship has been witnessed during
the programme. New advancements on the treatment of diabetic foot infection, offloading technique for the management
of diabetic foot ulcer and Charcot foot has been observed. The role and scope of practice for podiatrists was discussed.
Professional knowledge exchange platform was established. Overall, the programme was fruitful to enhance participants’
clinical skills in assessing and managing the diabetic foot. In this presentation, experience and insights gained on diabetic
foot management in UK will be shared.

Wednesday, 4 May Got an inspiration after visiting “Stroke Rehabilitation Unit” in Taoyuan Chang Gung Memorial Hospital, Taiwan

Visiting a rehabilitation service outside Hong Kong is only not a way of broadening the experience of stroke rehabilitation, but
also enhancing the criticism of the current practice. By attending the ‘Overseas Corporate Training Program in Rehabilitation
Nursing at Taoyuan Chang Gung Memorial Hospital’, it provides opportunities in comparing the practice’s similarities,
differences, banalities and commonalities. It has inspired the visiting team to understand the management and function of
other stroke rehabilitation care at other rehabilitation institution.

Taoyuan Chang Gung Memorial Hospital (TYCGMH) is managed by a non-profit foundation since 1976 under the National
Health Insurance (NHI) program held by Taiwan Government. It has been providing the stroke rehabilitation services to the
surrounding districts of Taoyuan and Taipei.

We visited the stroke rehabilitation service from 6.10.2014 to 31.10.2014. The journey aimed at learning the care delivery
model for rehabilitation nursing, the implementation of evidence based practice and the role of rehabilitation nurse in another
healthcare system.

During these four weeks, we visited different departments in understanding the care pathway of the stroke rehabilitation from
going through different stage of the illness from the acute hospital, rehabilitation unit, day rehabilitation centre, nursing home
care and the community service.

We were also privileged to witness the National Biotechnology and Medical Care Quality (nursing category) campaign. This
was a great event both to the institution and the service recognition at Taiwan.

At last, we are glad to have the opportunity to visit the Taoyuan Chang Gung Memorial Hospital. Visits will always have a
beneficial impact, perhaps often long term and indirect, on our ability to deliver and maintain a high standard of care for the
future.
