Stereotactic Spinal Radiation Therapy — Teamwork Approach in HA Settings

Spinal metastasis is a common clinical scenario in oncology. A significant proportion of spinal metastases may cause
symptoms or even devastating complications to cancer patients. Conventional radiation therapy remains the most commonly
employed treatment for symptomatic spinal metastases, and is able to control symptoms in patients with limited survival
chance. However, for patients with expected long survival, higher radiation dose is needed for durable local disease control.
This higher dose, ablative radiation therapy can be achieved with advanced radiation technique known as “stereotactic body
radiation therapy (SBRT)”.
Multidisciplinary approach is essential for the implementation of spinal SBRT programme. In this presentation, experience
and treatment results of Tuen Mun Hospital spinal oncology team which started service in 2013 will be introduced. It
represents the joint effort of orthopaedic surgeons, neurosurgeons, radiologists, clinical oncologists, medical physicist
and radiographers. Potential patients for spinal SBRT are reviewed in team meeting. Oncologists review SBRT feasibility,
overall cancer treatment plan and prognosis. Surgical teams assess the mechanical instability and operability. Radiologists
provide detailed radiological assessment based on images from computed tomography and magnetic resonance imaging.
Multidisciplinary approaches enable timely delivery of complicated treatment plan and maximise the use of resources-
intensive treatment technique.
