A group of three occupational therapists had the precious opportunity to attach to the three-week Medical and Geriatric,
Multidisciplinary Management Programme in Toronto Rehabilitation Institute. The programme aims to observe the daily
practice, acquire skills and learn the new management approach, for both professional development and enhancement of
related services in Hong Kong. Toronto Rehabilitation Institute provides various high quality programmes specially cater for
elderly patients with various disabilities. All the programmes are run by professionals from multidisciplinary; the healthcare
system adopts the Professional and Programme Structure Model of Care, with different roles and responsibilities for
different purposes. The Service Coordinator of different specialties is responsible for gate-keeping referral from different
acute hospitals and monitoring patient progress according to their protocol and guidelines. The Advance Practice Leader
is responsible for fostering and supporting best practice in the specialty and conducting continuous education and training
to further enhance the quality of patient services. The person taking up different roles, such as the above mentioned, could
be of different professional background. This certainly strengthens the important roles of allied health professional in the
rehabilitation setting.
The Profession and Programme Model adopted in Toronto Rehabilitation Institute is proved to be an effective and innovative
model which further enhances patient-centred care, with evident-based practices, staff training and education being the
focus.
