An Education Programme on Indwelling Urethral Catheter Care for Healthcare Workers in a Medicine and                             Monday, 18 May

Introduction

Catheter-associated urinary tract infection (CAUTI) is the most common hospital-acquired infection accounting up to 30 to
40% in hospital settings. If the use of catheter is deemed necessary, keeping up the day-to-day nursing care standard for the
catheter system shall become the best fundamental preventive measure of CAUTI. Effective training should be provided for
healthcare workers who are dedicated to bedside nursing care.

Objectives

The aim of the programme is to empower healthcare workers in caring patients with indwelling urethral catheter (IUC) in the
Medicine and Geriatric Unit. The objectives are: (1) to promote perineal hygiene in less independent patients with IUC; (2) to
increase healthcare workers’ knowledge on IUC; and (3) to enhance healthcare workers’ skills in caring patients with IUC.

Methodology

An education programme comprising of theoretical and practical parts was conducted in Cantonese from September to
November 2014. The theoretical part included the anatomy of male and female urinary system, the principle of indwelling
urethral catheterisation, and the appropriate care for the IUC and drainage system. It was then followed by a practical session
of meatal care for both gender through demonstration and return demonstration on manikins. Two assessment methods
were employed to evaluate the effectiveness of the programme. They were the pre- and post-written tests evaluating the
learners’ theoretical knowledge; and the clinical examination on caring one male and one female in-hospitalised patients with
IUC. These learners were given opportunities to re-seat the theoretical class and re-take the examinations.

Results

13 female healthcare workers in the unit participated in this education programme. All of them were Chinese-literate with
primary to secondary education level. The result has shown significant statistical improvement on written tests scores from
65.4% to 83.1% (p=0.022, Wilcoxon Signed Ranks test). In clinical examination, participants scored an average of 94.0%
and 94.8% in female and male catheter care respectively. This education programme was considered effective to increase
healthcare workers’ knowledge on IUC and enhance their skills on caring the catheter system. It is highly suggested that a
yearly refresher training should be conducted.
