Objectives
As an integral part of the hospital service delivery, the Medical Social Service Department which is working in a multi-disciplinary team, has to cooperate and collaborate with other professionals, such as medical, nursing and allied health and community so as to provide holistic care to patients. The medical social workers play an important role in the patients¡¦ total rehabilitation by
Assisting patients and their family members with social and emotional problems involved in illness situation
Enabling patients to make the best use of medical/rehabilitative services available
Contributing to the total rehabilitation of patients and to facilitate their reintegration into the community
Scope of Service
Provide psycho-social assessment and intervention
Provide counseling service
Provide pre-admission and discharge planning service
Coordinate and mobilize the community resources to resolve patients¡¦ problems e.g. application for financial assistance, home help service, placement¡K.etc.
Organize educational/ therapeutic groups/ programs on the relevant services for patients, carers and community
These services are provided through: interviews, home visits (if necessary), multi-disciplinary case conferences, educational talks, therapeutic groups, collateral collaboration

Introduction
The Patient Resource Centre of Tung Wah Hospital was opened in November
1995 in order to complement the Hospital Authority¡¦s strategy of
¡¥Comprehensive Health Care Service¡¦ and to align with Tung Wah
Hospital¡¦s spirit ¡¥To serve the Community¡¦. The Centre is managed by
Medical Social Service which provides relevant health information and
organizes activities directed to patients, their families and the community.
It aims at reducing their illness-related anxieties and to increase the health
awareness of the general public.
Objectives
To provide enquiry services, health information, education and other relevant resources to the public so as to increase their health awareness.
To enhance the quality of life for chronically ill patients and to promote the spirit of mutual and self-help among themselves.
To
promote volunteer services in and outside the hospital.
To improve inter-disciplinary cooperation among hospital staff and improve communication between the community and the hospital.
