服務宗旨

瑪麗醫院病人資源中心是聯繫醫院與社區夥伴、病人及照顧者的橋樑,為有需要的病人和照顧者作出支援。

核心服務包括:

● 病人與照顧者賦能及支援
● 支援病友組織
● 義工服務及發展
● 社區協作及建立夥伴關係


為配合病人於社區的需要,病人資源中心透過與相關臨床醫護團隊合作,推動病人及照顧者賦能,並協助病人與病友組織聯繫,達致相互支持及加強自我照顧,對提升病人管理健康的能力擔當重要角色。

此外,義工參與及與不同社區持份者協作,對支援病人在治療歷程中的多方面需要亦十分重要。為此,醫管局的病人資源中心致力發展病人義工服務以支援病人,並建立與社區夥伴的聯繋網絡,讓病人在社區保健安康。

3. 作為病人、照顧者、義工、社區機構的橋樑及推動者,建立彼此的夥伴關係


Mission

Patient Resource Centre (PRC) of Queen Mary Hospital (QMH) serves as the hospital focal points in engaging the community partners, and liaising with our patients and their carers for enhancing support according to patient needs.

The core services of PRC involve the following areas:

● Empowerment and support for patients and carers
● Support to patient groups
● Volunteer services and development
● Community engagement and partnership


To better meet the needs of our patients in the community, PRC plays significant roles in supporting patients to manage their health through patient and carer empowerment in collaboration with respective clinical teams as well as connecting patients with patient groups for mutual support and self-care.

In addition, volunteer participation as well as collaboration with different community stakeholders are also important to meet the multi-faceted needs of patients along patient journey. In this regard, PRC strives for the development of volunteer services for patient support in HA as well as building up network with community partners to facilitate our patients stay healthy in the community.

1. To promote psychosocial support and community integration for patients and carers
2. To establish platforms for patient and carer empowerment and self-management
3. To act as an interface and enabler in engaging patients, carers, volunteers and community organizations as partners in healthcare
