Please return the completed form and donations (cheque or original bank-in-slip) to North District Hospital, 9 Po Kin Road, Sheung Shui, N.T., H.K. Please mark “North District Hospital Charitable Foundation” on the envelope. Donation methods are as follows:

2. One-off Donation

     Donation by crossed cheque should be made payable to “North District Hospital Charitable Foundation”

     Deposit donation to North District Hospital Charitable Foundation Bank of China bank account.
