Community Service Programs in Fung Yiu King Hospital
   
Hong Kong West Community Geriatric Assessment Team (HKW CGAT)
  The Hong Kong West CGAT is a multidisciplinary team based at FYKH. The members of the team include Geriatricians, Community Visiting Medical Officers (CVMOS), Community Care Nurses, Occupational Therapists, Physiotherapists, Speech Therapists, Dietitians, Medical Social Workers and clerical staff. Key objectives for CGAT services are to provide timely assessment and appropriate management of health problems for elderly persons living in residential care homes (RCHEs); to improve the interfacing between medical and social services for elderly persons; to develop and provide community based rehabilitation program for elderly persons; to ensure decisions for elderly persons in need of Central Infirmary Services are appropriate; to promote quality of care of elderly persons through training and education; and to provide end of life services to selected RCHEs.
   
Community Visiting Medical Officer (CVMO)
  Two CVMOs are providing timely management of the ad hoc medical problems in older people living in selected RCHEs with a view to enhancing the general health status of the older RCHE residents and at the same time reducing unnecessary emergency attendances and hospital admissions. The CVMOs also actively participate in all infection control activities in RCHEs, including the yearly Winter Surge Program.
   
Winter Surge Program
  HKW CGAT has implemented the annual Winter Surge Program during the peak winter season of influenza-like-illness. It aims at preventing infection outbreak, enhancing infection control in RHCEs, reducing unnecessary emergency attendances and hospital admissions during the high risk period.
   
Enhanced CGAT Services for End of Life Care in Residential Care Homes
  It aims at providing comfort care for older residents in aged homes who have irreversible chronic medical diseases or cancers and to establish Advanced Care Planning with patients and family members. It also helps to equip CGAT, aged home and ward staff the essential knowledge and skill for EOL care and to ensure the older people can enjoy a dignified and comfortable period in their familiar environment before at their last phase of their life journey.
