Please complete and return the registration form together with a cheque of the exact amount to the Convention Secretariat at Room 219N Hospital Authority Building, 147B Argyle Street, Kowloon, Hong Kong SAR. All cheques should be made payable to “Hospital Authority”.

Deadline for registration is 15 April 2009. Thereafter, delegates have to register on site in the Registration Counter of the Hong Kong Convention and Exhibition Centre on 4 or 5 May 2009 with an additional fee of 20%.
