Granny Kwok, who is in her 80s and confined to bed, suffered from various stages of pressure injuries in different body areas. Her sacral wound condition was the most serious, with the appearance of necrotic tissue. She was referred to PMH CNS upon discharge. Lam Man, Registered Nurse (CNS), understood how worried and anxious Kwok’s family was. Every time when she conducted home visits, she would make use of the ‘Wound App’ to show them the wound healing progress as well as empowering them on wound care and health management. “They were relieved when seeing her wound size reduced from seven centimeters to three centimeters within three months, which is a demonstration of the effectiveness of patient engagement and shared care.”
Lam Man shows the progress of wound care to Granny Kwok’s sister and brother-in-law with the app. They were both surprised by the app’s illustration of the before-and-after comparison of the wound which could help them understand Kwok’s condition and explain it to their family members overseas easily, relieving the worries of all.
