Palliative Home Care Service
The palliative home care service consists of a multidisciplinary team of medical, nursing, medical social workers, psychologist and volunteers that provides home visits, outpatient clinics and telephone consultations for patients in their own homes. The aim is to maximize the patients' quality of life and lessen their suffering so that both the patients and their relatives are able to treasure the remaining days with hope and meaning.
Whom do we help?
We provide care for patients with cancer which is too advanced for attempts of curative treatments, and also for non-cancer patients with progressive incurable illnesses such as renal failure and neurological degenerative disease.
How to get service?
Referral can be made by the patientˇ¦s attending doctor.
What does our service include?
- Control of pain and other distressing symptoms by the use of drug and also non-drug measures.
- Social support by assessing the needs and concerns of caregivers and mobilizing necessary resources.
- Practical advice and support for caregivers in providing appropriate care for patients as well as for the caregivers themselves.
- Psychological support by allowing the patients to ventilate their fears and anxieties related to their illness.
- Spiritual support by addressing the patientsˇ¦ genuine needs and spiritual concerns.
- Bereavement support.
Office Hours
Office hours: Monday ˇV Friday from 8:45am to 5:45pm.
Telephone consultation: Everyday from 9am to 10pm
Charges
Service charges follow those recommended by the Hospital Authority.
