Department profile

As an integral part of seamless healthcare delivery service, we provide comprehensive and integrated community services to clients in their own environment with the aim of maximizing or maintaining their optimal level of self-care and functioning including physical and psychological well-being.

Our services include:

1. Community Nursing Service: specialized home nursing care to clients of all ages
2. Integrated Care and Discharge Support for Elderly Patients: pre-discharge planning and post-discharge support to high-risk elderly
3. Community Geriatric Assessment Service: outreach medical consultation and assessment to elderly living in the residential care homes
4. Community Allied Health Service: outreach rehabilitation service to maximize clients’ physical function and speed up recovery
